One word ends more NHS interviews than any clinical question ever will.

It isn't Care. It isn't Compassion. It isn't even Competence.

It's Courage — and the reason it trips up genuinely excellent clinicians has nothing to do with a lack of it.

In plenty of the systems we trained in, challenging a senior doctor carries risk. Seniority means authority. Authority isn't questioned out loud. You learn, early, to work within that.

The NHS was built to run on the opposite instinct.

After Mid Staffordshire, where a culture of silence let poor practice go unchallenged until patients were harmed, the NHS didn't just apologise. It legislated. Freedom to Speak Up. Duty of Candour. DATIX. A Speak Up Guardian in every Trust. All of it exists because silence, in that inquiry, was named as a cause of death.

So when a panel asks about Courage, they're not asking whether you've survived a hard shift. They're asking one thing: will you challenge unsafe practice, through the right channel, without waiting for permission?

Here's the shape of an answer that actually scores: name the concern, name the private, professional conversation you had first, name the escalation when it wasn't resolved, and close with why silence was never neutral — it was a risk to the patient in front of you.

That single answer touches Courage, Governance, Patient Safety and NMC alignment at once. Most candidates never build it, because nobody explains what the word actually means in this context.
