Whenever incidents like this occur, the first and most important step is to thoroughly investigate what happened. That means gathering accounts from the victims, eyewitnesses, and those involved in the rescue operation. From experience, we know that these investigation reports are often not properly analysed or integrated into operational planning, making it more difficult to prevent similar incidents in the future.
