The Medical Training (Prioritisation) Act came into force in March 2026, restricting access to NHS specialty training posts for international medical graduates in favour of UK and Irish graduates.

What it does not affect: Trust Grade SHO, Clinical Fellow, Locally Employed Doctor, and Staff Grade/Specialty Doctor posts — all non-training posts that remain fully open to international graduates.

For international doctors, the practical route into NHS medicine currently runs through these non-training posts, which require GMC registration with a licence to practise and a strong, NHS-relevant application.
