You Have Been Working As An NHS Locum For Six Months. Here Is How To Convert That Into A Substantive Post — And Why The Application For A Locum Is Different From The Application For A Permanent Role.

Locum work in the NHS is one of the most underutilised springboards into substantive employment for internationally trained doctors.

Not because it guarantees a permanent offer. It does not.

But because it generates something that six months of overseas applications never can: UK-validated, NHS-documented, consultant-supervised clinical evidence.

Six months of NHS locum practice gives you:

Named NHS consultant referees who have directly observed your practice in a UK clinical environment. This is the single most powerful reference type for any subsequent substantive application.

UK clinical governance participation — Datix reports, ward rounds, handover, MDT documentation, discharge summaries in the NHS EPR format. Every one of these is CESR portfolio evidence if you are pursuing Specialist Registration.

NHS-specific procedural evidence — cannulation, lumbar puncture, central line insertion, ABG, ward assessment — documented in the UK clinical record. Deanery assessors and CESR reviewers recognise this specifically.

UK workplace-based assessment opportunities — mini-CEX, CbD, DOPS — if your supervising consultant is willing and your Trust supports this.

The challenge: most locum doctors do not capitalise on any of this because they are treating the locum post as temporary employment rather than as a strategic credential-building opportunity.

Here is what to do differently:

In your first week, identify and approach a consultant willing to be your named educational supervisor. Even informally. Even without a formal agreement. One WBA per month, signed by a GMC-registered consultant, is transformative for your next application.

Document everything. Not just your clinical work. Your governance participation. Your audit contributions. Your MDT inputs. Your escalation decisions.

When you apply for the substantive post — at the same Trust or elsewhere — your supporting statement leads with your NHS locum experience as the primary evidence of UK clinical competence. Not your overseas background. That becomes supporting context.
