The NHS Pay Question Every International Nurse Is Asking In 2026. The Honest Answer Nobody Gives You.
What band will I start on when I join the NHS?
I hear this every week from nurses who have registered with the NMC, passed OSCE, achieved their OET or IELTS, and are now applying for NHS jobs.
And the honest answer — which almost no recruiter gives you — is: it depends on four things that most overseas nurses do not negotiate.
One — Your NMC registration. Full NMC registration as a Registered Nurse qualifies you for Band 5 minimum under Agenda for Change pay scales. The 2025/26 pay award placed Band 5 entry at approximately £28,407 rising to £34,581 at the top of the band. This is the base. This is not negotiable downward.
Two — Your years of post-qualification experience. NHS Trusts are not legally required to recognise overseas post-qualification experience for pay banding purposes — but many do under AfC provisions. The key word is "recognise." A Trust that does not have a formal policy for assessing overseas experience may default to Band 5 entry regardless of whether you have 10 years of specialist nursing experience. You must ask — in writing — during the offer process: "Does this Trust assess overseas post-qualification experience for AfC banding purposes?"
Three — Your specialty. Band 6 clinical specialist nursing posts in critical care, emergency, theatre, neonatal, and community nursing are actively advertising. These are not Band 5 roles with a Band 5 salary. They are specialist roles requiring demonstrable post-qualification specialist experience. If your clinical background maps directly to a specialist area, applying for Band 5 may be underselling your profile.
Four — Your documents. A Band 5 application that does not demonstrate specialist competency will not convert to a Band 6 offer. A Band 6 application built with generic NHS language and no person specification mapping will not shortlist. The pay outcome you achieve is directly related to the grade you successfully apply for — and that is determined by your documents before it is determined by your experience.
