"The Clinical Error Question"
"Tell us about a time you made a mistake."
Your heart rate jumps. Your mind goes blank. And then you say:
"I always double-check my work, so I have not really made a significant clinical error."
The panel marks you down immediately. Not because they think you are incompetent. Because they now know you are either dishonest — or dangerously unaware of your own fallibility.
And both of those are patient safety risks.
Let me tell you something the NHS takes more seriously than almost any other professional value:
Duty of Candour.
It is a legal obligation under the Health and Social Care Act 2008. It means that when something goes wrong — you acknowledge it, you are honest about it, and you take action to ensure it does not happen again.
Every registered clinician in the UK is bound by it. And every NHS interview panel, at every band, is assessing whether you have internalised it.
The clinician who says "I have never made a mistake" fails the Duty of Candour assessment on the spot.
The clinician who says "Here is what went wrong, here is what I did immediately, here is what I changed, and here is the evidence that the system is now safer" — scores Outstanding.
Here is the framework I use with every client I prepare for this question:
The PDSA Cycle — Plan, Do, Study, Act.
— Plan: What was the situation and what went wrong?
— Do: What did you do immediately — disclosure, escalation, patient safety action?
— Study: What did you reflect on? What did you learn? What did the incident reveal about the system?
— Act: What specific change did you implement, recommend, or contribute to — that made the system safer?
That is not a confession. That is a clinical governance narrative. And it is the most powerful thing you can bring into an NHS interview room.
Your past error — framed correctly — becomes your strongest evidence of professional maturity, patient safety commitment, and NHS Values alignment.
