"The Safety Filter"
There is one question in every NHS Band 5, 6, and 7 interview that functions as a hard filter.
Pass it — and the rest of the interview opens up.
Fail it — and it does not matter how well you answered everything else.
It is the deteriorating patient scenario. And in 2026, there is only one acceptable answer framework.
ABCDE.
Airway. Breathing. Circulation. Disability. Exposure.
Not because it is the only way to assess a deteriorating patient — you know that. But because it is the structured, verbalised, evidence-based approach that tells an NHS panel you are safe to practise on a UK ward without supervision.
And it does not stop at ABCDE.
You must reference NEWS2 — the National Early Warning Score — as your deterioration recognition tool. You must demonstrate that you know when and how to escalate using SBAR. You must show that you understand the escalation chain — from bedside nurse to senior clinician to rapid response team — and that you would never delay that escalation to protect your own confidence.
This is what the panel is scoring: not clinical knowledge. Safety culture.
Every internationally trained clinician I have prepared for NHS interview has walked into that deteriorating patient question knowing exactly what to say, in what order, and why each element scores points on the assessment matrix.
The ones who wing it — even the ones with years of ICU experience — consistently underscore on this question because they answer it clinically rather than structurally.
The structure is the answer.
