What caused so many states to fail
Across the country, the audit failures were driven by:

18 states not publishing audit reports for state accounts.

21 states not publishing local government audit reports.

Weak or inactive public accounts committees only 3 states had effective ones

Poor implementation of audit autonomy laws.
