I empathize with this family and I hope something positive will come out of this tragedy.
1. That this child was critically ill implied more caution was needed not less
2. Even a normal dose of propofol could cause respiratory arrest more so in an unstable child- absence of monitoring(Pulse-ox, ETC02) was a critical error
3. Why is central line, LP, MRI, ordered? this exposed a critical child to avoidable risk. Close knowledge & skills gap in our hospitals
